Notice for Intent to Issue Corrigendum to the RFQ and
Extension of Deadline for Submission of Applications
Development of Diagnostic Imaging Centre at IbniSina
HospitalCompound (Kabul) by PPP using BOT Model

Ministry of Public Health (MOPH), Islamic Republic of Afghanistan, intends to develop a Diagnostic Imaging Centre at Ibni Sina Hospital Compound by PPP using Build-Operate-Transfer(BOT) model and invitedinterested companies/consortia on 20thAugust 2019 via Notice for Prequalification. Pre-Application meetings were held on 21st September 2019, 12th October 2019, 10th November 2019 and 20th November 2019.
Following feedback received from interested companies/consortia, the MOPH is revising the scope and transaction structure to make the projectfinancially viable and bankable. Upon concluding the assessment, the MOPH will publish a Corrigendum to the RFQ (a replacement version of the RFQ)through website (www.mof.gov.af) and will share it through Email with Registered Entities.
The deadline for submission of Applications is hereby also extended to 29thFebruary 2020 to give sufficient time for the issue for the Corrigendum by the MOPH and completion the Application Forms and compilation of supporting documents by Registered Entities. It is expected the assessment of scope of work and transaction structuring, and issue of the Corrigendum to the RFQ will take four to six weeks.

Notice for Extension of Deadline for Submission of Applications
Development of Diagnostic Imaging Centre at Ibni Sina
Hospital Compound (Kabul) by PPP using BOT Model

Ministry of Public Health (MOPH), Islamic Republic of Afghanistan, intends to develop a Diagnostic Imaging Centre at Ibni Sina Hospital Compound by PPP using Build-Operate-Transfer (BOT) model and invited interested companies/consortia on 20th August 2019 via Notice for Prequalification. Pre-Application meetings were held on 21st September 2019, 12th October 2019, 10th November 2019 and 20th November 2019.
It should also be noted that interested companies/consortia can still register their interest by sending the completed Registration Form to ppphealth.moph@gmail.com to become Registered Entities, to be eligible to submit Applications to participate in the bidding process.
The deadline for submission of Applications is hereby also extended by a further two weeks to give sufficient time to complete the Application Forms and compile supporting documents. Applications must be submitted in hard copy on or before 1400 hours AFT on 21st December 2019 to the following address. Applications submitted electronically will not be accepted.

Extension of Deadline for Submission of Applications

Ministry of Public Health (MOPH), Islamic Republic of Afghanistan, intends to develop a Diagnostic Imaging Centre at Ibni Sina Hospital Compound by PPP using Build-Operate-Transfer (BOT) model and invited interested companies/consortia on 20th August 2019 via Notice for Prequalification. Pre-Application meetings were held on 21st September 2019, 12th October 2019 and 10th November 2019.
Registered Entities and other interested companies/consortia are hereby notified that a 4th Pre-Application meeting will be held on 20th November 2019 at MOPH to address different issues and queries raised by the potential Applicants.
It should also be noted that interested companies/consortia can still register their interest by sending the completed Registration Form to ppphealth.moph@gmail.com to become Registered Entities, to be eligible to submit Applications to participate in the bidding process.
The deadline for submission of Applications is hereby also extended by a further three weeks to give sufficient time to complete the Application Forms and compile supporting documents following the 4th Pre-Application Meeting. Applications must be submitted in hard copy on or before 1400 hours AFT on 7th December 2019 to the following address. Applications submitted electronically will not be accepted.

Previous Notice
Ministry of Public Health (MOPH), Islamic Republic of Afghanistan, intends to develop a Diagnostic Imaging Centre at Ibni Sina Hospital Compound (Kabul) by PPP using Build-Operate-Transfer (BOT) model and hereby invites interested companies/consortia to submit Applications in response to the Prequalification Document, so that it may evaluate and shortlist those Applicants who shall be invited to respond to the RFP. As part of the RFP Stage, MOPH will evaluate the Proposals submitted by the Shortlisted Bidders and appoint Preferred Bidders to undertake the Project.
Interested companies/consortia can register their interest by sending the completed Registration Form to ppphealth.moph@gmail.com to become Registered Entities, allowed to submit Applications to participate in the bidding process. The Registration Form and Prequalification Document can be accessed below. A Pre-Application meeting will be organized on 17th September 2019 at MOPH, details of which would be duly notified to the Registered Entities.

Form 6 – Letter of Authorisation for Lead Member of Consortium
Form 7 – Authorisation to a representative
Form 8 – Certificate of Compliance
Form 9 – Certificate by Affiliate of Applicant or Consortium Member of Willingness to Participate
Form 10 – Prequalification Response Checklist
Applications must be submitted in hard copy on or before 1400 hours AFT, on 15th October 2019 to the following address. Applications submitted electronically will not be accepted.
